Our Responsibilities

Quantum Cellular Therapies LLC (“Quantum Cellular Therapies,” “we,” “our,” or “us”) is required by law to:

- Maintain the privacy and security of your protected health information (PHI).

- Provide you with this Notice of our legal duties and privacy practices.

- Notify you promptly if a breach occurs that may have compromised the privacy or security of your PHI.

- Access Your Records: Request to see or obtain a copy of your medical record and other health information we have about you.

- Request Corrections: Ask us to correct health information you believe is inaccurate or incomplete.

- Request Confidential Communications: Ask us to contact you in a specific way (e.g., only at home or by mail).

- Request Restrictions: Ask us not to use or share certain PHI for treatment, payment, or operations. (We may not be able to agree to all requests.)

- Get a List of Disclosures: Request a list of certain disclosures we have made of your PHI.

- Receive a Paper Copy: Request a paper copy of this Notice, even if you agreed to receive it electronically.

- Choose Someone to Act for You: If you have given someone medical power of attorney or have a legal guardian, that person can exercise your rights.

- File a Complaint: If you believe your privacy rights have been violated, you can file a complaint with us or with the U.S. Department of Health and Human Services (HHS). We will not retaliate against you for filing a complaint.


Our Uses and Disclosures That Require Your Authorization

We will not use or disclose your PHI for marketing purposes, sell your PHI, or share psychotherapy notes without your written authorization. If you provide an authorization, you may revoke it at any time.

Changes to This Notice
